- After registration, you will receive your card typically in two weeks.
- Your state issued card is valid for 365 days and costs 75$.
- Its your responsibility to maintain this card online with state.
Step#2: Get Your Certificate from Dr. Farooq
- On your First Visit Dr. Shiraz Farooq will evaluate if you qualify to be certified and upon qualification you will receive your orders (script) for 70 days.
- After 70 days you will have your 1st Tele-Visit where you will receive your orders (script) for 70 more days. Make sure you book your appointment timely in advance.
- After another 70 days you will have your 2nd Tele-Visit when you will receive your orders (script) for another 70 days.
- After another 70 days (on 210th day from your evaluation) you will have your 3rd Tele-Visit to get Recertified and get orders for next 70 days.
