Crohn’s Disease is a perplexing and often misunderstood condition that affects the gastrointestinal tract, the long tube that runs from the mouth to the anus. It is a type of inflammatory bowel disease (IBD) and affects hundreds of thousands of people in the United States. Understanding Crohn’s Disease is crucial for effective management and treatment.

Key takeaways

  • Crohn’s disease causes patches of inflammation that can appear anywhere in the digestive tract. It most often affects the end of the small intestine and the colon.
  • Common symptoms are diarrhea, belly pain, tiredness and weight loss. Symptoms come and go in flares.
  • It is diagnosed with a mix of blood and stool tests, colonoscopy with biopsies and imaging.
  • There is no cure yet, but medicines, diet changes and sometimes surgery can control it well.
  • Crohn’s can also cause painful problems around the anus, such as fistulas and abscesses, which a colorectal surgeon treats.

What are the Symptoms of Crohn’s Disease?

Understanding the Variability of Symptoms: Crohn’s Disease symptoms can vary significantly from person to person. Some may experience mild symptoms, while others face severe and debilitating effects. This variability makes it essential to recognize the different ways in which the disease can manifest.

Recognizing Common Symptoms: The most common symptoms include diarrhea, abdominal pain, and fatigue. These symptoms often develop gradually but can also appear suddenly. It’s important to be aware of these signs as early recognition can lead to timely treatment.

Periods of Remission and Flare-ups: Crohn’s Disease is characterized by periods of remission, where symptoms may disappear, and flare-ups, where symptoms become more pronounced. Understanding this cycle is key to managing the condition effectively.

Symptoms Beyond the Gut: Crohn’s is not only a bowel problem. Inflammation can affect other parts of the body, and some people notice these signs first:

  • Weight loss without trying, or poor growth in children
  • Low-grade fever and night sweats during flares
  • Mouth ulcers
  • Painful or swollen joints
  • Red, painful eyes or blurred vision
  • Tender red bumps or sores on the skin
  • Pain, swelling or discharge around the anus

When to Get Urgent Care: Go to an emergency room if you have severe belly pain, vomiting that will not stop, a swollen belly with no gas or stool passing, heavy bleeding from the rectum, or a high fever. These can be signs of a blocked bowel, an abscess or another serious complication.

Is it Genetic?

The Role of Genetics in Crohn’s Disease: Genetics can influence the likelihood of developing Crohn’s Disease, especially if there is a family history of the condition. However, most cases occur without a known genetic link, indicating other contributing factors.

Other Contributing Factors: In addition to genetics, environmental factors and lifestyle choices, like smoking, play a crucial role in the development and severity of Crohn’s Disease.

How Serious is it? Is it Fatal?

The Impact of Crohn’s on Quality of Life: While Crohn’s Disease can be severe and sometimes lead to life-threatening complications, with appropriate treatment, many people lead fulfilling lives. Understanding the potential impact on quality of life is important for managing expectations and treatment goals.

Managing Complications: Complications such as bowel obstruction, malnutrition, and an increased risk of colon cancer can arise. Regular monitoring and treatment adjustments are vital in preventing and managing these complications.

How is Crohn’s Disease Diagnosed?

No single test confirms Crohn’s disease. Doctors put together several pieces of information:

  1. History and exam: your doctor asks about your symptoms, family history and medicines, and examines your belly and the area around the anus.
  2. Blood tests: these look for anemia, signs of inflammation and infection, and low vitamin levels.
  3. Stool tests: these rule out infection and can measure inflammation in the bowel.
  4. Colonoscopy with biopsies: a thin camera looks at the colon and the end of the small intestine, and small tissue samples are taken. This is usually the key test. Learn more about colonoscopy at ColoWell America and the diseases a colonoscopy can detect.
  5. Imaging: CT or MRI scans of the small intestine (enterography) show areas a colonoscopy cannot reach, along with narrowing, fistulas or abscesses. Some people also need an upper endoscopy or a capsule camera.

Some symptoms of Crohn’s overlap with irritable bowel syndrome. IBS does not cause inflammation or damage to the bowel, which is why testing matters.

How is Crohn’s Disease Treated?

Treatment aims to calm the inflammation, keep you in remission and prevent complications. Most people need a combination of approaches, and the plan changes over time.

Medication: Several types of medicine are used, depending on where the disease is and how active it is:

  • Steroids calm a flare quickly but are only used for short periods because of their side effects.
  • Immunomodulators such as azathioprine or methotrexate lower the immune response over the long term.
  • Biologic medicines, given by injection or infusion, target specific parts of the immune system and are often used for moderate to severe disease.
  • Newer oral medicines are an option for some people.
  • Antibiotics may be used for infections, abscesses or fistulas.

Diet and Nutrition: No diet causes or cures Crohn’s, but food can make a difference to how you feel. During a flare, smaller meals and softer, lower-fiber foods are often easier to tolerate, especially if part of the bowel is narrowed. Drink plenty of fluids if you have diarrhea. Keeping a food diary can help you spot your own triggers. Crohn’s can make it hard to absorb nutrients, so some people need vitamin or iron supplements, or special liquid nutrition, with the help of a dietitian.

Surgery: Many people with Crohn’s need surgery at some point. It is considered when medicines are not controlling the disease, or for complications such as a narrowed bowel (stricture), a blockage, a fistula, an abscess or a hole in the bowel. The surgeon removes or widens the damaged section and saves as much healthy bowel as possible. Surgery does not cure Crohn’s, and it can come back in another area, so medical treatment usually continues afterwards.

Crohn’s Disease and the Anus: Fistulas and Abscesses

Crohn’s disease often affects the area around the anus. This is called perianal Crohn’s disease, and it is an area where a colorectal surgeon plays a central role.

  • Abscess: a pocket of pus near the anus. It causes a painful, swollen, warm lump, and sometimes fever. An abscess needs to be drained, so see a doctor the same day or go to urgent care.
  • Fistula: a small tunnel that forms between the inside of the bowel and the skin near the anus. It can leak pus, blood or stool and keep coming back. Learn more about anal fistula treatment.
  • Fissures and skin tags: small tears in the lining of the anus and soft flaps of skin are also common with Crohn’s.

Perianal Crohn’s is usually treated with a combination of medicine and surgery, such as draining an abscess or placing a soft drain called a seton through a fistula. Getting the medical and surgical teams to work together gives the best chance of healing.

Crohn’s Disease vs. Ulcerative Colitis

Crohn’s disease and ulcerative colitis are both types of inflammatory bowel disease, and they share many symptoms. The main differences are:

  • Where it happens: Crohn’s can affect any part of the digestive tract, from mouth to anus. Ulcerative colitis affects only the colon and rectum.
  • Pattern: Crohn’s causes patches of inflammation with healthy bowel in between. Ulcerative colitis causes continuous inflammation that starts in the rectum.
  • Depth: Crohn’s can go through the full thickness of the bowel wall, which is why it can cause strictures and fistulas. Ulcerative colitis affects the inner lining.
  • Typical symptoms: both cause diarrhea and belly pain. Bleeding and urgency are more typical of ulcerative colitis. Weight loss and problems around the anus are more typical of Crohn’s.
  • Role of surgery: in ulcerative colitis, removing the colon can cure the bowel disease. In Crohn’s, surgery treats complications but the disease can return.

Don’t Wait, Your Health Matters

The Importance of Early Detection: Early detection of Crohn’s Disease through symptoms recognition and diagnostic procedures like colonoscopies can significantly improve treatment outcomes.

Frequently Asked Questions (FAQs)

The first signs are often diarrhea that keeps coming back, belly pain or cramping, tiredness and weight loss. Some people first notice mouth ulcers, joint pain, or pain and swelling around the anus. If these last more than a few weeks, see a doctor.

Flares often happen without a clear reason. Common triggers include missing doses of medicine, smoking, infections and anti-inflammatory painkillers such as ibuprofen. Stress and certain foods can make symptoms worse for some people.

Not yet. Crohn’s is a lifelong condition, but treatment can control it and keep it in remission for long periods. Many people with Crohn’s work, travel and live full lives.

Smaller meals of softer, lower-fiber foods, such as white rice, bananas, eggs, cooked vegetables without skins and lean protein, are often easier during a flare. Drink plenty of fluids and limit greasy food, spicy food and alcohol if they bother you. A dietitian can help you plan meals that keep your nutrition up.

Crohn’s can affect any part of the digestive tract in patches and can go through the full bowel wall. Ulcerative colitis affects only the colon and rectum, in one continuous area of the inner lining. Both are types of inflammatory bowel disease.

Crohn’s that affects the colon raises the risk of colorectal cancer, especially when it has been present for many years. That is why people with Crohn’s colitis are advised to have regular surveillance colonoscopies. Read more about colorectal cancer.
